We are open Tuesday through Saturdays for appointments. Please click here to see our complete office hours. A comprehensive health history questionnaire along with other forms can be picked up or mailed to you for completion prior to your first visit. Please plan to drop off the forms before the day of your visit, fax them to 503-771-1660, or arrive 20 minutes early to complete the forms in our office.
